Job Overview

Position:
Intensive In-Home Associate III
Department: BH - Qualified Professional

Schedule:

Monday‑Friday 9:00am – 5:00pm (40 hours/week)
Pay:
Minimum $22.00 per hour

Qualifications
  • Minimum of a Bachelor’s degree (Human Services preferred) or higher, depending on experience.
  • Valid U.S. driver’s license.
  • Qualified Professional (QP) – Monarch‑DSM certification.
  • 1–4 years of experience working with children or adolescents with mental health, intellectual/developmental disability, or substance use disorders.
  • For QP 10A NCAC 27G .0104:
    Bachelor (non‑Human Services) with 4yr full‑time or 8yr part‑time experience

    • Bachelor (Human Services) with 2yr full‑time or 4yr part‑time experience

    • Master (Human Services) with 1yr full‑time or 2yr part‑time experience
Responsibilities
  • Coordinate and oversee initial and ongoing assessment activities with the Team Leader.
  • Convene the Child and Family Team for person‑centered planning.
  • Develop, revise, and implement the Person‑Centered Plan.
  • Consult with medical and non‑medical providers, engage community supports, and integrate input into planning.
  • Ensure linkage to appropriate services, including psychological and psychiatric evaluations.
  • Assist families in accessing community resources such as recreation, housing, food, clothing, school programs, vocational opportunities, and life‑skills services.
  • Provide and coordinate behavioral health services and interventions for youth and other family members with licensed professionals and team members.
  • Monitor and document progress and effectiveness of interventions.
  • Assist with crisis interventions and provide 24/7/365 on‑call support to families.
  • Maintain medical and service records in compliance with state, federal, and agency guidelines.
  • Participate in educational trainings and maintain required certifications and competencies.
  • Drive to community locations and arrange transportation as needed.
  • Engage in ongoing quality improvement activities and follow service definition guidelines.
  • Support positive relationships with community partners, agencies, licensing bodies, and funding resources.
