Home Health & Hospice Central Intake SC currently seeks a Full-Time (Non-Clinical)
Referral Specialist that oversees all of South Carolina.
The Referral Specialist serves as a liaison between the Agency and the medical community to coordinate patient care. They ensure patient care activities are appropriate, timely and cost effective.

- Maintains e-referral systems for hospital systems, physician practices, SNFs, and any other referral source sending electronic referrals
- Maintains weekly referral logs noting NTUC reasons to be shared with ADM, DOPS and/or DOM weekly
- Answers e-referrals timely, both acceptance and denials
- Maintains frequent communication with ADM, DOPS and/or DOM regarding referral patterns and/or trends
- Acts as a liaison between the field staff and physicians in obtaining plans of care and appropriate modified orders.
- Receives information from field staff regarding patient care issues. Prioritizes and reports these issues to the appropriate parties.
- Reviews findings with the admitting nurse of requested services.
- Participates in start of care conferences.
- Recognizes problem areas and facilitates group problem solving.
- Ensures that physician’s orders are signed in a timely manner.
- Attends regularly scheduled meetings with physicians and/or appropriate designees to review patient care issues.
- Participates in and conducts joint projects and presentations with other community resources to promote community awareness.
- Attends agency staff meetings and agency and/or non-agency continuing education programs.
- Participates in the agency’s quality/process improvement program.
- Completes community education as needed.
- Contributes to team effort by accomplishing related results as needed.
- High school diploma or GED required. Bachelor’s degree preferred.
- One year home care experience preferred.
- Experience promoting community awareness as it relates to home health and/or hospice preferred.
