Home Health Nurse - RN (Open)
The Home Health Nurse has primary accountability for nursing coordination and provision of individualized care for patients of all ages. They utilize delegation and critical thinking skills to provide quality service through a team-oriented, collaborative approach to patient care. The role is full-time on-call, with weekend coverage. The nurse works independently in a variety of home settings and is responsible for assessing, planning, implementing, and evaluating patient care to achieve optimum outcomes.
This includes age-specific admission and ongoing assessments, development and revision of interdisciplinary plans of care, accurate medication administration, documentation in electronic medical records, and patient/family education. The nurse demonstrates clinical expertise, manages resources, establishes priorities, facilitates problem-solving, participates in new employee orientation, and upholds confidentiality. Physical requirements include lifting, prolonged standing, bending, pushing, pulling, reaching, and walking. The nurse may be asked to provide care at other facilities and may be required to take call.

- Graduate of a professional nursing school.
- One (1) year experience in hospice, home health, oncology, or medical-surgical nursing.
- Current Registered Nurse license in the Commonwealth of Virginia.
- Valid Virginia driver’s license and successful completion of a criminal record check.
- AHA BLS-HCP certification.
- Ability to complete orientation and competency requirements.
- Keyboarding skills for use with automated computer systems, including electronic medical records.
- Ability to meet essential job functions.
