Overview

As part of the healthcare team, the Home Health Nurse assumes primary accountability for nursing coordination and individualized care for patients of all ages, utilizing delegation and critical thinking within a collaborative approach. The role includes a flexible schedule with an initial orientation period (Monday–Friday) for 4–6 weeks, followed by a two‑day‑a‑week schedule.

Key Responsibilities
  • Assume primary accountability for nursing coordination and provision of individualized care for patients of all ages.
  • Assess, plan, implement, and evaluate patient care to achieve optimum patient outcomes.
  • Complete age‑specific admission and ongoing assessments, document and use the data to develop a plan of care.
  • Collaborate with the healthcare team, patients, and families to develop, implement, and evaluate an interdisciplinary plan of care; revise the plan as patient status changes.
  • Accurately administer treatments and medications and document all in electronic medical records.
  • Identify patient/family educational needs, incorporate them into the care plan, and implement and document educational interventions.
  • Perform within clinical, leadership, and educational pathways to support evidence‑based, quality patient care service and performance improvement.
  • Exhibit clinical expertise in rapidly changing circumstances and plan strategies to reduce length of stay and improve patient outcomes.
  • Serve as a role model for clinical practice to coworkers, students, and physicians.
  • Manage resources, establish priorities, and modify the environment to meet patient care needs.
  • Complete manager/director predetermined projects within specified time frames and guidelines.
  • Use problem‑solving methods to facilitate resolution of patient and personnel situations.
  • Participate in the orientation of new employees.
  • Interact with patients, families, visitors, and all members of the health care team to develop, coordinate, and manage the plan of care; delegate and oversee delivery of patient care.
  • Maintain patient, staff, and customer confidentiality.
  • Demonstrate knowledge of life‑span growth and development, assess data reflective of the patient’s status, and provide care according to departmental policies and procedures.
  • May be exposed to infectious diseases, bio‑hazardous and hazardous materials, regulated medical waste, and combative patients.
  • Perform in a fast‑paced environment requiring prioritization skills.
  • Meet physical requirements: lifting, prolonged standing, bending, pushing, pulling, reaching, walking, etc.
  • May be required to provide care at other Carilion facilities or take call.
Qualifications
  • Graduate of a professional nursing school (Registered Nurse).
  • At least 1 year of experience in hospice, home health, oncology, or medical‑surgical nursing.
  • Licensed Registered Nurse in the Commonwealth of Virginia; must have a valid Virginia driver’s license and pass a criminal record check.
  • Americans Heart Association Basic Life Support – Health Care Providers (AHA BLS‑HCP) required.
  • Successfully complete orientation and competency requirements.
  • Keyboarding skills required for automated computer systems, including electronic medical records.
  • Must be able to meet essential job functions.
