RN Case Manager – Home Health (Full Time, Day Shift, United States)

We are seeking a registered nurse to manage home health and hospice patients across Lexington. The role involves assessment, care planning, coordination, and quality improvement to achieve optimal patient outcomes.

• Assess, develop, implement, and evaluate patient care plans for patients of all ages.
• Conduct age‑specific admission and ongoing assessments, document findings, and develop individualized plans of care.
• Administer and document treatments and medications via automated computer systems and electronic medical records.
• Provide education to patients, families, and caregivers and incorporate educational needs into care plans.
• Collaborate with interdisciplinary team members, scheduling, billing, and government agencies to ensure continuity of care and compliance.
• Perform skilled visits, including OASIS and recertification visits, and update case‑management worksheets.
• Advocate for patients, protect patients’ rights, and provide psychological, social, and cultural support.
• Lead performance‑improvement activities, manage resources, prioritize tasks, and modify environment to meet patient needs.
• Support orientation and supervise LPNs and Home Health aides as needed.

• Graduate of an accredited registered nurse program.
• Licensed RN in Virginia (or an approved compact state) with a valid driver’s license and personal vehicle for home visits.
• Demonstrated keyboarding skills for electronic medical record systems.
• Strong critical‑thinking skills and knowledge of patient‑care regulations (CHCS, Carilion policy).
• Experience with lifespan growth and development assessment and ability to adapt care to age‑specific needs.
• Ability to prioritize and work in a fast‑paced patient‑care environment.

Hourly range: $33.52 – $50.28 (does not include benefits).
