LPN Part-time, PRN

We are looking for a Home Health Licensed Practical Nurse (LPN) to join our team Full‑time and help our patients remain comfortable, at home.

This is a great opportunity for a Licensed Practical Nurse with home health experience who loves the life‑changing impact your nursing work has on your patients!

You Will:

• Provide nursing care under the direction of the Home Health Registered Nurse and the established Plan of Care to achieve positive patient outcomes.
• Communicate with family members and other members of the patient's multi‑disciplinary team.

What We Need from You:

• A commitment to helping our home health patients remain comfortable, at home.
• Proof of graduating from an accredited school of practical nursing.
• Current LPN license from the State of Ohio.
• Previous experience in home health care setting preferred.
• Superior organizational and time management skills.
• Reliable transportation, with a valid driver's license, insurance and registration to travel in assigned territories.
• Current CPR certification.
• Demonstrated compassion, empathy and strong customer service skills.
• A completed and successful TB test, physical examination and a criminal background check.
