MDS COORDINATOR (RN/LPN)

Avalon View Health and Wellness is seeking an experienced and detail-oriented MDS Coordinator to join our clinical team in Liberty, Missouri.

The MDS Coordinator is responsible for coordinating the Resident Assessment Instrument process and supporting the accuracy, timeliness, and integrity of MDS assessments, care plans, Medicare documentation, and clinical reimbursement processes.

This position works closely with nursing, therapy, dietary, social services, medical providers, and other members of the interdisciplinary team to ensure each resident’s needs, services, goals, and clinical conditions are accurately assessed and documented.

ABOUT THE POSITION

The MDS Coordinator oversees the timely and accurate completion of the facility’s MDS and Resident Assessment Instrument processes. This includes coordinating assessment schedules, reviewing supporting clinical documentation, monitoring Medicare requirements, participating in care planning, and helping ensure services are accurately reflected for clinical and reimbursement purposes.

The MDS Coordinator serves as an important resource for the interdisciplinary team and helps maintain compliance with applicable federal, Missouri, Medicare, Medicaid, and facility requirements.

RESPONSIBILITIES
  • Coordinate the Resident Assessment Instrument and MDS processes for facility residents
  • Develop and maintain an interdisciplinary schedule for MDS assessments and care plan reviews
  • Ensure MDS assessments are completed accurately and within required time frames
  • Establish and communicate Assessment Reference Dates in collaboration with the interdisciplinary team
  • Coordinate completion of required assessment sections by appropriate members of the interdisciplinary team
  • Review the medical record to ensure documentation supports MDS coding, resident conditions, treatments, services, and functional abilities
  • Monitor documentation for accuracy, completeness, consistency, and regulatory compliance
  • Work with nursing, therapy, dietary, social services, activities, and other departments to obtain necessary assessment information
  • Participate in daily clinical meetings, stand-up meetings, Medicare meetings, and other resident review processes as assigned
  • Help identify significant changes in resident condition and determine whether an assessment or care plan update is required
  • Coordinate with the Director of Nursing or designee regarding physician orders, clinical reports, and resident changes
  • Ensure the care plan reflects identified resident needs, preferences, risks, strengths, and measurable goals
  • Facilitate interdisciplinary care plan meetings and encourage resident and responsible-party participation
  • Monitor Medicare eligibility, skilled coverage, certification requirements, and supporting documentation
  • Help ensure continued-stay documentation is completed accurately and within required time frames
  • Support accurate clinical reimbursement and Patient-Driven Payment Model processes
  • Review assessment information for potential coding discrepancies before submission
  • Track MDS submissions, corrections, modifications, accepted assessments, and completion deadlines
  • Complete regular audits of MDS coding, care plans, and supporting clinical documentation
  • Report identified trends, risks, and audit findings to nursing leadership and the Quality Assurance and Performance Improvement Committee
  • Provide MDS, documentation, and care-planning education to clinical personnel as needed
  • Stay informed about changes to MDS, RAI, Medicare, Medicaid, and clinical reimbursement requirements
  • Assist with documentation preparation for regulatory surveys, audits, and medical record reviews
  • Maintain resident privacy, confidentiality, and protected health information
  • Perform other duties related to the position as assigned
QUALIFICATIONS
  • Graduate of an approved practical or registered nursing program
  • Current, active, and unencumbered RN or LPN license in the State of Missouri
  • Current BLS/CPR certification
