Skilled Nursing Facility | 6:00a–6:00p and 6:00p–6:00a Shifts Available

$10,000 Sign-On Bonus – Full-Time Only

Liberty Health Care Center is seeking dependable, compassionate LVNs and RNs to join our skilled nursing team.

Your Responsibilities
  • Shift Leadership & Coordination - Receive and provide thorough shift reports; review MAR/TARs, new orders, and safety alerts
  • Shift Leadership & Coordination - Prioritize assignments, support CNAs, and ensure safe, efficient delivery of care
  • Shift Leadership & Coordination - Perform scheduled and PRN rounds; support new admissions and transitions
  • Medication & Treatment Administration - Complete medication passes accurately and on time, following the Five Rights
  • Medication & Treatment Administration - Administer treatments and PRN medications per provider orders
  • Medication & Treatment Administration - Manage controlled substance counts, reorders, and documentation per policy
  • Clinical & Skilled Nursing Duties - Provide wound, respiratory, urinary, and enteral care within the scope of license and training
  • Clinical & Skilled Nursing Duties - Perform IV therapy if certified; obtain specimens and monitor vital signs and condition changes
  • Clinical & Skilled Nursing Duties - Notify providers and families of significant changes; document clearly and on time
  • Resident Experience & Teamwork - Partner with the DON, ADON, RN Supervisor, and interdisciplinary team to meet resident goals
  • Resident Experience & Teamwork - Communicate with residents and families in a compassionate, professional manner
  • Resident Experience & Teamwork - Assist with ADLs as needed — teamwork always comes first
  • Compliance & Safety - Maintain accurate EMR documentation and support survey readiness
  • Compliance & Safety - Follow infection control practices and protect resident dignity and confidentiality
Qualifications
  • Active, unencumbered Texas LVN or RN license (required)
  • CPR/BLS certification (required); IV Therapy certification preferred
  • Knowledge of SNF/LTC regulations, standards of care, and documentation requirements
  • Strong clinical judgment, time management, and teamwork skills
  • EMR experience required (Point Click Care  preferred)
  • Physical ability to lift 60–70 lbs, stand/walk for extended periods, and assist residents safely
  • Reliable attendance, positive attitude, and resident-centered mindset
  • Must meet all pre-employment requirements (background check, drug screen, TB, and health clearances)
