Duties and Responsibilities- Complete nursing assessment forms in a timely manner.
- Supervise home health aides every two weeks.
- Identify physical, emotional, and developmental needs of clients.
- Develop individual nursing care plans reflecting current client needs prior to case conference.
- Provide health teaching to clients and families.
- Maintain accurate clinical records of home health services.
- Assess client condition on every home visit.
- Communicate assessments to appropriate team members.
- Implement nursing interventions to meet client needs and changing conditions.
- Document clinical records per agency policies.
- Transcribe, implement and evaluate the physician’s plan of care timely.
- Implement infection control and safety measures.
- Monitor client compliance with care plans.
- Perform procedures such as IV therapy, catheter insertion and irrigation, wound care, and wound vacs.
- Follow agency policy, procedures, and regulatory requirements.
- Graduate of an accredited nursing school.
- Registered Nurse licensed and passed the state board exam.
- Experience in home health or public health nursing preferred.
- Knowledgeable about current nursing trends and legislative issues.
- Excellent verbal and written communication skills.
- Strong administrative, organizational, and leadership skills.
- Adequate vision and hearing to comprehend written and verbal communication.
- Ability to lift heavy objects, bend, and stand for extended periods.
- Ability to manage stress effectively.
- Document nursing process and patient responses accurately.
- Maintain confidentiality in accordance with HIPAA.
- Provide professional, respectful care and establish rapport with patients.
- Submit visit notes and reports on time.
- Participate in care conferences and in‑service programs.
- Visit cases punctually and notify patients before scheduling visits.
- Cooperate and communicate with the health team.
- Engage in educational opportunities to enhance practice.
- Perform other duties as assigned.
