Lambs Farm in Libertyville, IL seeks a dedicated Case Manager to coordinate services for individuals in our ICF/IID program. You will advocate, develop and monitor IPPs, Behavior Support Plans, and collaborate with a multidisciplinary team to ensure participant rights, safety, and independence.
You will maintain timely documentation, attend annual IPP meetings, accompany participants to appointments, and support families and guardians while upholding privacy and regulatory standards.
