Capstone Hospice in Georgia is seeking an experienced RN Case Manager to provide compassionate, end-of-life care. You will assess patients, develop individualized care plans, manage pain and symptoms, and educate families about the hospice process.
You will work Monday through Friday, 8:00 am to 5:00 pm, with IDG meetings every other week in the office. The role requires strong communication, emotional resilience, and the ability to balance a caseload of 10–14 patients while maintaining HIPAA
