Social Services Director

Care Core at Lima is seeking a compassionate and experienced Social Services Director to lead our social services department. Our 88-bed skilled nursing facility is committed to providing the highest quality of care and support to our residents and their families. We’re looking for someone who is passionate about making a difference in the lives of others and thrives in a collaborative team environment.

• Develop and implement social service programs that meet the emotional and social needs of residents
• Provide support and counseling to residents and families
• Coordinate discharge planning and community resources
• Maintain documentation and records in accordance with state and federal guidelines
• Serve as an advocate for residents and ensure their rights are protected
• Work closely with interdisciplinary team members to promote resident well-being
• Participate in care plan meetings and contribute to care planning

• Current, active Social Work license in the state of Ohio (Preferred but not required)
• Minimum of 2 years experience in a long-term care or healthcare setting
• Strong leadership, communication, and organizational skills
• Knowledge of applicable state and federal regulations
