As a Hospice Patient Care Coordinator (RN) you will be called to care when you're needed most. As part of Interim Health Care, you'll provide oversight of the care/services furnished by employees to Interim Health Care Hospice patients.

As a Hospice Patient Care Coordinator (RN), you will:

• Manage referrals
• Serve as an expert for the hospice admission process and perform admissions as assigned
• Collaborate with the Director of Clinical Services to develop and implement processes to ensure the efficient and effective achievement of patient/client outcomes in accordance with physician orders

To qualify for a Hospice Patient Care Coordinator (RN) with us, you will need:

• Graduate of an accredited school of nursing and licensed to practice as an RN in the state in which employed. License is active and in good standing.
• Two (2) years of experience as a Hospice RN required and at least on year of supervisory experience preferred
• Able to provide proof of valid driver's license and auto liability insurance in accordance with policy
