Functioning within the Health System’s mission, values, objectives, procedures and policies, the Oncology and Infusion Clinical Pharmacist works with other members of the health care team to ensure appropriate drug therapy is being utilized while optimizing pharmaceutical care for the patient. This position promotes comprehensive pharmaceutical care for patients and supports a multidisciplinary approach to care through participating in medication therapy decision making processes, recommending and implementing modifications to therapy based on patient needs and monitoring parameters, collaborating with the team to update plans in the EHR ensuring accuracy and addressing drug interactions, and documenting pharmacist interventions.
This position also supervises the duties of the Pharmacy Technicians and verifies the accuracy of their work. The Oncology and Infusion Clinical Pharmacist makes therapeutic drug decisions as requested by the medical staff with regard to the various clinical pharmacy services, including formulary management, medication shortage guidance, and provider order set creation, evaluations, and updates. Assists with the compliance with regulatory requirements of the Joint Commission, the Ohio Board of Pharmacy, and all federal regulatory agencies.
Responsible for the timely completion of project work as deemed appropriate by the Clinical Pharmacy Supervisor. Assists in the development and execution of pharmacy services and competency testing for the pharmacy staff. Serves as the primary pharmacist for a nursing unit or service line, including attending designated committees. Precepts pharmacy students and residents as required.
Graduate of an accredited Pharmacy program with a minimum of a Bachelor’s degree is required.
Licensure/CertificationA valid unrestricted license from the Ohio State Board of Pharmacy is required. Must meet all continuing education requirements for re-licensure with the Ohio State Board of Pharmacy. Certification in Oncology (BCOP) by the Board of Pharmaceutical Specialties is required within 12 months from eligibility, and certification must be maintained throughout the length of employment in position.
ExperienceA minimum of two (2) years of experience in a health care setting with a focus on Oncology and Infusion Services is required or completion of a PGY1 residency.
SkillsMust demonstrate strong verbal, clerical, and numerical abilities. Must have a strong clinical pharmacy knowledge base and experience in clinical pharmacy services. Must be acquainted with the distributive systems of the Pharmacy Department, including the unit dose drug distribution system, the IV admixture system, and the computerized order entry/verification system.
