Job Description- Participates directly or indirectly in the development of the Individual Treatment Plan (ITP) for patients (e.g., provides input on the plans designed around the habilitation and rehabilitation in cooperation with the multidisciplinary treatment team)
- Recommends and works in collaboration with treatment team on changes in treatment plans as behavior shows progress or regression and carries out patient or group programming delineated from the multidisciplinary treatment team
- Trains patients to function independently in self-help and daily living skills
- Completes activity of daily living for those unable to perform tasks (e.g., hygiene, dressing, feeding/eating, grooming, moving/lifting patients and toileting to include application/changing of incontinence pads/briefs and bedding
- Provides preventative skin care and physical shower assistance
- Measures height/weight
- Assists in social skills such as writing/written communications, identifying name and addresses, budgeting, and time telling by using a variety of training techniques
Directs and monitors vocational/educational responsibilities
- Assists in escorting patients and/or accompanies to scheduled meetings (e.g., nurse sick call, doctor’s sick call, education and job related)
- Performs specific tasks with patient as assigned by supervisor and/or multidisciplinary treatment team (e.g., vocational and programming task)
- Assists in the planning of and implementation of leisure/recreational activities (e.g., games, gardening, library visits, recreational yard/activities, commissary, religious programming, community service) and documents the activities and outcomes in progress notes.
- Institutes appropriate crisis intervention for unacceptable behavior as defined by treatment plan and prevents injury of patient to self or others
- Completes necessary documentation of incident reports as required
- Monitors on and off unit surveillance of patient behavior
- Investigates patient noncompliance with unit rules
