POSITIONSUMMARY
The Care Transitions Nurse is a vital part of the Hospice team who will be responsible for conducting hospice eligibility assessments, providing direct patient care, providing education to patients, family and applicable staff members, and acting as the primary point of contact to ensure coordination and continuity of care as patient lifecycle progresses. This position will work closely with Hospice team members, as well as members of the Company’s Home Health Agency.

- Must be in good physical condition as the position requires the ability to stand on one’s feet throughout the day.
- Must be able to complete performance requirements independently while using good judgment as well as have the ability to harmoniously work with others.
- Must be able to read, write and understand the English language.
- Must be a graduate of an accredited Nursing School.
- Must possess an active Registered Nurse License within the applicable state.
- Must have a minimum of 2 years nursing experience in Home Health, Hospice or Skilled Home Care (Hospice preferred).
- Reliable transportation to/from work locations.
- Valid driver’s license and clean motor vehicle record.

- Prior hospice experience with a deep understanding of Medicare regulations and eligibility requirements for hospice care.
- Previous experience in sales/marketing preferred, but not required.
- Ability to market and deal tactfully with customers and community.
- Ability to demonstrate self‑confidence and positive attitude toward self and others and maintain commitment and enthusiasm to goal achievement.
- Must be adaptable, patient‑focused, and capable of handling challenging situations with tact and empathy.
- Will have access to patient medical records and patient financial accounts as well as personnel records of department staff, and thus must be knowledgeable on HIPAA regulations.
- Ability to communicate well, both verbally and in writing.
- Knowledge of Microsoft Office 365 and Agency’s EMR system (Kan Time) preferred.
- Knowledge of administrative and clerical procedures.
