Licensed Practical Nurse (LPN)

As a Licensed Practical Nurse (LPN) at our healthcare team, you will play a crucial role in delivering comprehensive nursing care to our residents. You will work under the supervision of registered nurses and other healthcare professionals to ensure the well-being and comfort of our residents. Your dedication, clinical expertise, and compassion will directly impact the lives of those in your care.

Responsibilities
  • Perform routine nursing tasks under the guidance of registered nurses, physicians, and other healthcare professionals.
  • Administer medications and treatments as prescribed by the healthcare team.
  • Monitor and assess residents' vital signs and condition, reporting any changes promptly.
  • Assist with resident admissions, transfers, and discharges, ensuring proper documentation and communication.
  • Provide wound care, injections, and other medical procedures as needed.
  • Collaborate with the interdisciplinary team to develop and update individualized care plans for residents.
  • Assist in maintaining a clean and safe environment for residents and staff.
  • Document all nursing care provided accurately and in a timely manner.
  • Educate residents and their families on healthcare procedures and provide emotional support.
  • Respond to emergencies and provide first aid when necessary.
  • Adhere to all state and federal regulations, as well as facility policies and procedures.
  • Participate in ongoing education and training to maintain licensure and stay current with best practices in nursing.
Qualifications
  • Valid Licensed Practical Nurse (LPN) license in state of practice.
  • CPR certification required.
  • Previous experience in a skilled nursing facility or long-term care setting is preferred.
  • Strong communication and interpersonal skills.
  • Ability to work as part of a team and independently as needed.
  • Compassion, patience, and a commitment to providing exceptional care.
  • Basic computer skills for electronic health record documentation.
  • Excellent organizational and time-management skills.
  • Flexibility to work various shifts, including weekends and holidays as needed.
