Role Overview:
This position is for a Registered Nurse (RN) providing in-home patient care, assessing, planning, and implementing individualized care plans.
Responsibilities:Conduct patient assessments, educate patients and families on health conditions, procedures, and medications, and manage complex needs such as wound care, tracheostomies, G-tubes, catheters, ventilators, and medication administration.
Qualifications & Skills:Open to new graduates, with training programs including lab, shadowing, and hands-on instruction. Strong clinical skills, adaptability, and excellent communication are essential.
Key Duties:Deliver personalized patient care, build long-term relationships, and support patient independence and health milestones. Flexibility to create a suitable schedule and access to 24/7 clinical support.
