ISN Personal Agent

Position: Full‑time professional – Personal Agent
Location: Lincoln City, OR (serving Tillamook and Lincoln Counties)
Salary: $25.08 – $27.91 per hour
Schedule: Monday – Friday, 8:30 am – 5:00 pm
Type: Full‑time, Non‑Exempt

General Position Summary

The Person al Agent provides trauma‑informed case management services to persons with intellectual and/or developmental disabilities, using person‑centered practices to assist individuals in identifying support needs and life goals. The agent guides and supports clients in accessing services to achieve those goals.

Responsibilities
  • Conduct individual and group meetings in clients’ homes or community locations.
  • Develop individualized service plans (ISP or Annual Plan) based on assessments.
  • Assist clients to plan, implement, and monitor services and goals.
  • Provide information, education, and technical assistance (rights, choice, transition planning, complaint resolution).
  • Authorize services in the Department’s electronic payment and reporting system.
  • Maintain up‑to‑date case notes and claims documentation on a weekly basis.
  • Share business calendar with coworkers and attend team meetings and community events.
  • Participate in protective services as required by state regulations.
  • Coordinate and support Friendship Connections and community resource development.
  • Conduct assessments using approved tools (e.g., Foster Care Support Needs Assessment) when not previously provided case management within last six months.
  • Review brokerage records and conduct case‑note audits to ensure compliance with regulations.
  • Stay current on federal, state, and local laws, policies, and procedures relevant to case management.
  • Travel to counties served by ISN; occasional overnight stays may be required.
  • Use Microsoft Office applications (Word, Teams, Excel) and other automated systems for work assignments.

  • Complete projects within specific time limits.
Preferred Qualifications
  • Prior experience in Person‑Centered Planning and Self‑Determination.
Education and Experience Requirements
  • Bachelor’s degree in behavioral science, social science, or a closely related field.
  • Associate degree in behavioral science, social science, or a closely related field plus 2 years of human services experience.
  • High school diploma or GED plus 4 years of human services experience.
  • Knowledge of Oregon public service systems.
  • Experience working in a team setting.
  • Reliable transportation and valid Oregon driver’s license (or ability to obtain within 30 days of hire).
  • Proficiency in Microsoft Office (Word, Teams, Excel).
