PART I: ESSENTIAL POSITION FUNCTION AND DUTIES
Provides individual, group and family therapy. Participates in interdisciplinary treatment planning. Develops an annual comprehensive psychosocial assessment. Provides crisis intervention consultation to all members of the team. Participates in peer‑review process. Provides pre‑screening for depression, anxiety and other psychological disorders. Conducts individual and family counseling sessions with clients. Diagnose psychological disorders. Conducts psycho‑social assessment. Provides case management. Assists clients as they apply for health insurance.
Contacts social service agencies on behalf of clients to obtain services. Makes referrals to other agencies and psychiatrist as appropriate. Schedule patients with psychiatrist and preps patient chart for visit. Completes electronic charting in EMR. Attends integrated meetings. Completes superbills, electronic billing. Performs other duties as assigned. Continuously looks for and suggests ways to improve. Effectively completes assignment to achieve the greatest benefits at acceptable cost.
Implements improvements as appropriate. Demonstrates interest in own growth and development by periodically evaluating own performance, demonstrating an awareness of personal abilities and limitations, independently seeking means to make improvements. Attends and participates in in‑services and continuing education programs.
- Any fully licensed mental health professional (i.e., LMSW, LPC) to provide mental health and/or behavioral health services in the State of Michigan with a minimum of two years experience.
- Limited licensed mental health professional who are in the process of obtaining full licensure within 6–12 months. Documentation is needed upon interview.
SKILLS AND ABILITIES
- Knowledge of health care administration systems.
- Knowledge of governmental regulations and reimbursement requirements.
- Knowledge of computer, systems, and applications.
- Skilled in planning, organizing and supervising patient care.
- Skilled in exercising initiative, judgment, problem‑solving, decision‑making.
- Skilled in developing and maintaining effective relationships with medical and administrative staff, patients and the general public.
- Proficiency in Microsoft Excel and knowledge of other Microsoft Office programs.
- Skilled with working in a complex work environment.
- Ability to maintain confidentiality at all times and maintain organizationally appropriate relationships.
- Knowledge of organization policies and procedures.
- Presents a positive image of Western Wayne Family Health Centers at all times.
- Carries out assignments with little need for direction.
- Timeliness.
- Provides proper notification of absence and tardiness.
- Works weekends and shifts when necessary.
