Position Summary

The Emergency Community Support Specialist provides and coordinates rehabilitative and support services to individuals involved or at risk of being involved in the Emergency Behavioral Health system. Program services are provided 24‑hours per day, 365 days a year. Estimated length of service per client is 90 days with the possibility of extensions available until long term supports are in place.

Job Duties
  • Develop with clients integrated treatment team plans, updating every month or as needed.
  • Facilitate implementation of clients’ goals, including but not limited to community living skills, daily living skills, interpersonal skills, vocational and educational skills, medication management, physical health care, mental health services and care, substance abuse services, resource acquisition, psychiatric emergency and relapse needs, etc.
  • Discuss discharge goal with client and develop a transition plan.
  • Conduct required assessments.
  • Provide and arrange client transportation to program-related appointments as needed.
  • Arrange for and/or personally provide transportation for clients to various behavioral health, medical, and community appointments that facilitates the treatment plan.
  • Develop a crisis response and safety plan for each client. Assess for client safety as needed and respond to crisis and safety concerns.
  • Coordinate all treatment activities with services provided by community agencies, treatment providers, or by another resource.
  • Advocate for the client and partner with other service providers.
  • Maintain a caseload while meeting program billing requirements monthly.
  • Attend all required meetings, trainings, and supervision sessions.
  • Participate in on call coverage as assigned to include holidays and weekends.
  • Document all services and communication per program guidelines.
  • Participate in the organization’s Quality Improvement (QI) activities as assigned.
  • Abide by HIPPA regulations and ensure confidentiality of clients and safe record keeping.
  • Work with collaborating partners of the TASC Program including but not limited to specialized response for the Crisis Center, Lincoln Public Schools, The Bridge Behavioral Health, Aging Partners law enforcement, etc.
  • Other duties as assigned by supervisor.

Education and Experience
  • High school degree is required. Bachelor’s degree or higher in Psychology, Sociology, or a related Human Service field preferred.
  • In lieu of a bachelor’s degree, two years’ experience with the following is acceptable – coursework in a Human Services field, work experience, training, or lived recovery experience with demonstrated skills and competencies in treating individuals with a behavioral health diagnosis.
  • Case management experience and crisis management experience required.
  • Basic understanding of physical health issues and impact on mental health/functioning preferred.
  • Demonstrated understanding of and ability to work with people of diverse backgrounds.
  • Demonstrated decision‑making skills and understanding of mental illness and impact on functioning.
  • Knowledge of community organizations and their function/purpose related to human services.
  • Valid driver’s license, liability auto insurance, and ability to drive a personal vehicle on a daily basis to transport clients safely within the community.
Physical Requirements
  • Hybrid work environment (in‑office, field, and remote).
  • Laptop and company issued cell phone.
  • Walking and transporting client to and from various locations in the community.
  • Valid driver’s license, proof of liability auto insurance, and ability to drive a personal vehicle on a daily basis for client and program needs within the community.
