Job Description
The Pharmacy Technician supports population health initiatives by optimizing medication use and improving patient outcomes within a specific group or community.
Please note that a video interview through Microsoft Teams will be required as well as potential onsite interviews and meetings.

Collaborates with pharmacists and other healthcare professionals to manage medications, improve adherence, and address medication-related issues that impact the overall health of a population.
Plays a crucial part in value-based care programs by monitoring medication adherence, identifying gaps in care, and supporting interventions aimed at improving health outcomes and reducing healthcare costs.
Assists in medication reconciliation, ensuring patients have accurate medication lists, especially during transitions of care.
Works to improve medication adherence by contacting patients, providing education, and addressing barriers to taking medications as prescribed.
Collects and analyzes data related to medication use, adherence, and outcomes to identify trends and areas for improvement within the population.
Collaborates with pharmacists, physicians, nurses, and other healthcare professionals to coordinate care and ensure patients receive appropriate medication-related services.
Educates patients and their families about medications, potential side effects, and the importance of adherence, often through phone calls or other communication methods.
Contributes to population health initiatives that focus on improving quality of care, reducing costs, and meeting performance metrics within value-based care models.
Generates reports on medication adherence, therapeutic outcomes, and other relevant metrics to track progress and identify areas for improvement.

Completion of pharmacy technician training program or at least two (2) years of experience as a pharmacy technician.
Current pharmacy technician certification and/or license in state of practice.
Pharmacy Technician Certification Board (PTCB) or another approved organization.
Experience in value-based care or medication management settings.
Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
May be expected to stand in a stationary position for an extended period of time.
For roles requiring driving:
Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
