At Community Alliance, we believe in the potential of recovery for all individuals with a mental illness and we dedicate our resources to support the journey of each individual we service.
Duties Include- Participate as a member of the Healthy Transitions Care Coordination Team, provide strategic outreach, engagement and care coordination for transition-age youth and young adults at risk for serious mental illness or serious emotional disturbance, with a focus on subgroups including individuals facing complex behavioral health needs, physical health conditions, and/or social determinants health. Will use own lived experience to engage and assist individuals in working toward their own recovery.
- The Peer Support Specialist will assist with system navigation for project participants, communicating regularly and proactively with the person served, family, and community partners; initiating, monitoring and follow-up of referrals and warm handoffs for behavioral health treatment, rehabilitation, and recovery services; in-reach to individuals before discharge, release or transition from other services or systems of care (e.g. hospital, children/juvenile justice systems to adult systems);
connection to specialty services such as FEP, Supported Education/Employment, Family Education and Support services; and facilitating identification and access to needed community services including housing, primary health care, education/vocational support, transportation, and benefits programs. - Knowledge, training, and skills of person-centered care, recovery model, motivational interviewing, community resources for adults and transition-age youth.
- Ability to work well and contribute within an inter-professional team; presents a welcoming and hopeful approach.
- Ability to communicate effectively both orally and in writing with persons of diverse backgrounds and perspective.
- Demonstrated cultural sensitivity and competencies.
- Bachelors degree in social work, rehabilitation, education or related field; or lived experience with mental illness and eligible to receive certification in Nebraska as Peer Support Specialist within 6 months of hire.
