Position Title: Group Home Team Lead
Reports to: Program Director or Residential Services Director
Location: Nebraska (multiple nearby group home sites)
Supervises: Direct Support Professionals (DSPs) across 3–5 homes

Role OverviewThe Group Home Coordinator ensures consistent, high‑quality residential supports across assigned homes. This role combines supervision of staff with oversight of client outcomes, adherence to ISPs, regulatory compliance, and community integration. The Coordinator guides daily operations, fosters safety, and supports a positive, growth‑focused environment.
Key Responsibilities 1. Supervision & Staff Leadership- Hire, train, mentor, and evaluate DSPs to ensure person‑centered care and adherence to best practices
- Create staff schedules, manage shift coverage, and approve timecards.
- Provide on‑call support and arrange emergency staffing as needed
- Lead weekly team meetings, in‑service training, and promote a culture of continuous improvement.
- Ensure delivery of services aligned with Individual Support Plans (ISP), including behavior support and habilitation programs
- Monitor DSP documentation—T‑logs, MARs, GERs, behavior data, communication logs—to meet state timelines and quality standards
- Conduct residential home visits to evaluate quality of life, environment, and compliance
- Facilitate annual ISP meetings and collaborate with interdisciplinary teams (families, guardians, therapists, DHHS) on goal setting and adjustments
- Advocate on behalf of clients for housing, benefits, medical care, and community resources.
- Assist with managing client personal finances, SSI reporting, and low‑income support services if desired
- Ensure homes and vehicles meet state and agency safety regulations, documentation standards, and regulatory requirements
- Monitor implementation of behavior plans; intervene in crisis situations using approved support strategies
- Report and respond to incidents promptly and accurately through regulators such as DHHS as required
- Maintain regular communication with leadership regarding operational issues, staffing, and client support needs.
- Support financial tracking, billing accuracy, and cost‑effective operation of assigned homes
- Use office tools (e.g. Word, Excel, Therap, N‑focus, etc.) for documentation, data entry, and communication
- Participate in agency intake, program review, and collaborative case meetings. Other duties as assigned.
- Bachelor’s degree preferred in social work, psychology, sociology, education, human services, or related field
- Equivalent experience considered
- Minimum 2 years in human services supporting individuals with developmental disabilities
- At least 1 year of supervisory or coordinative experience preferred
- Familiarity with ISP development, behavior support, and positive behavioral supports
- Valid Nebraska driver’s license, clean driving record, and insurance
- CPR, First Aid, Medication Aid certification preferred (or training provided)
- Strong written and verbal communication, interpersonal and time management skills
- Proficiency in Microsoft Office and documentation systems like Therap, N‑focus, or equivalent
- Commitment to person‑centered, inclusive support
- Detail‑oriented, organized, adaptable
- Professional demeanor; ethical, confidential, empathetic
- Primarily daytime work with some evenings and weekends as needed
- Regular visits to homes, flexibility for in‑home supervision shifts
- On‑call to support after‑hours emergencies
- Ability to lift up to 50–75 lbs, assist with physical interventions where necessary
