Tabitha's Home Health in York/Fillmore/Saline Counties, NE seeks a full‑time Physical Therapist Assistant (PTA) to work 8 a.m. – 4:30 p.m. Each PTA enhances the living experience for our seniors by providing physical therapy services under the direction of a licensed Physical Therapist. Services are delivered in nursing homes, home‑health, and other contracted facilities.

• Review seniors’/clients’ charts to obtain current information that may affect PT treatment; seek guidance from supervising PT.
• Recognize abnormal physiologic changes and report them to the supervising PT.
• Perform PT treatment procedures as delegated by the supervising PT in accordance with the initial evaluation and plan of care.
• Develop and implement the client’s plan of care in collaboration with the physician, other health‑care professionals, and family/caregivers per agency procedures and state/federal regulations.
• Actively participate in care conferences and provide input to the PT for plan development.
• Maintain a work schedule that allows coverage in all rehab programs as needed.
• Use strength, agility, stamina, and prioritization skills to respond promptly to frequent, overlapping client and caregiver requests.
• Maintain written documentation reflecting the department standard.
• Communicate effectively with families, caregivers, team members, and support services to achieve client goals and meet service expectations.

• Age 18 or older.
• Graduate of an accredited PTA program.
• One year of PTA experience (preferred) and geriatric experience (preferred).
• Physical Therapist Assistant license in good standing (Nebraska State Registry), CPR certification, valid Nebraska driver’s license, good driving record, and required auto insurance.
• All licenses, certifications, and permits kept up to date at the employee’s expense; compliance with Tabitha’s vaccination requirements.
