Hospice Aide – Hillcrest Hospice & Home Care

Location: Lincoln, NE
Service Line: Hillcrest Hospice & Hillcrest Home Care
Schedule: Part-Time

Hillcrest Health & Living is looking for a compassionate, dependable, and caring Hospice Aide to join our team supporting both Hillcrest Hospice and Hillcrest Home Care in the Lincoln area.

This unique part-time position will be split between hospice and home care
, giving you the opportunity to support patients and families across different stages of care while providing meaningful, one-on-one assistance in the comfort of their homes.

You will also serve as an important connection between patients, families, and the clinical team by observing and communicating changes in a patient's condition.

What You’ll Do

As a Hospice Aide
, you will provide personal care and supportive services according to each patient's individualized plan of care. Your time will be divided between patients receiving hospice services and those receiving home health services throughout the Lincoln area.

You will:

  • Provide compassionate, respectful personal care to patients in their homes.
  • Assist with bathing, grooming, dressing, oral care, shaving, hair care, nail care, and other personal-care needs.
  • Assist patients with toileting and other activities of daily living.
  • Help patients with ambulation, transfers, positioning, and wheelchair mobility according to the established plan of care.
  • Assist with meals and feeding when appropriate.
  • Obtain and document vital signs, including blood pressure, temperature, pulse, respirations, and weight as directed.
  • Provide hot and cold applications and non-sterile dressings when included within the approved plan of care and scope of practice.
  • Assist patients with medications that are normally self-administered, within the appropriate scope of practice and established guidelines.
  • Follow each patient's individualized plan of care as established by the clinical care team.
  • Observe patients for physical, emotional, behavioral, or functional changes and promptly communicate concerns to the appropriate nurse, case manager, or clinical leader.
  • Provide comfort-focused care and support to hospice patients while maintaining dignity and honoring individual preferences.
  • Offer a calm, reassuring presence to patients and families during sensitive and emotional situations.
  • Follow infection-control, OSHA, safety, and agency procedures.
  • Complete required documentation accurately and timely.
  • Properly use, clean, and store equipment and supplies after care is completed.
  • Communicate effectively with nurses, case managers, patients, families, and other members of the interdisciplinary care team.
  • Travel to patient homes throughout the Lincoln service area.
  • Assist with additional patient-care needs within the appropriate scope of practice as assigned.

The role requires a strong understanding of CNA scope of practice, individualized care plans, infection-control procedures, vital-sign monitoring, patient observation, and timely communication of changes to the clinical team.

The right candidate will understand that hospice care requires a special level of compassion and presence
. Sometimes the most important part of the job is helping someone feel comfortable, listening to a family member, protecting a patient's dignity, or simply being a calm and reassuring presence.

Because this position also supports Home Care patients, you will work with individuals who have a variety of needs and goals. You will need to be comfortable adjusting your approach based on each patient's condition, care plan, and individual preferences.

Ideal candidates will have:

  • A compassionate and respectful approach to caring for aging adults and individuals with serious or life-limiting illnesses.
  • Previous CNA, hospice, home health, senior living, skilled nursing, hospital, or direct-care experience preferred.
  • Experience providing personal care in a patient's home or similar environment preferred.
  • Comfort providing end-of-life and comfort-focused care.
  • Ability to support patients and families during emotional or sensitive situations.
  • Strong observation skills and the ability to recognize and promptly report changes in a patient's condition.
  • Strong communication skills with patients, families, nurses, case managers, and other care-team members.
  • Ability to follow individualized plans of care and remain within established scope of practice.
