Schedule: Full-time | Includes rotating Patient & Family Services on-call coverage

Hillcrest Health & Living is looking for a compassionate, experienced, and relationship-focused Hospice Social Worker to join our Hillcrest Hospice team.
In this role, you’ll help patients and families navigate the emotional, social, practical, and financial challenges that can come with serious illness and end-of-life care. Whether you’re helping a family understand their options, connecting them with resources, advocating for a patient’s wishes, or simply being present during a difficult moment, your work will make a meaningful difference.

Inspiring people to live their best lives. As a Hospice Social Worker, you’ll help ensure patients and families feel heard, supported, and cared for throughout their hospice journey.
As a Hospice Social Worker
, you’ll be an essential member of our interdisciplinary hospice team, providing psychosocial support, education, advocacy, care planning, and resource coordination for patients and families.
We’re looking for a Social Worker who brings both professional expertise and genuine compassion to patients and families during a deeply personal stage of life.
The right person will be comfortable navigating sensitive conversations, helping families through uncertainty, and balancing emotional support with practical problem-solving and resource coordination.
Qualifications include:
- Bachelor’s degree from an accredited Social Work program required.
- Master’s degree in Social Work preferred.
- At least two years of experience in Social Work, Case Management, or a related field.
- At least one year of social work experience in a healthcare setting required for candidates with a BSW.
- Hospice or home care social work experience preferred.
- CMSW and/or LMPH preferred.
- Strong psychosocial assessment, care-planning, and critical-thinking skills.
- Excellent communication and relationship-building abilities.
- Ability to navigate grief, loss, difficult decisions, and emotionally sensitive conversations with compassion and professionalism.
- Knowledge of community resources and experience connecting individuals and families with appropriate support.
- Strong organization, documentation, and follow-through.
- Ability to work independently in the community while remaining closely connected to the interdisciplinary team.
- Valid driver’s license, reliable transportation, and required automobile insurance.
