As a CNA at Lincoln Meadows Care Center, you will provide essential hands-on care and support to residents while maintaining dignity, comfort, and safety. You will work closely with the nursing team to deliver high-quality, resident-centered care in accordance with individualized care plans.
Key Responsibilities (Summary):- Assist residents with activities of daily living, including bathing, dressing, grooming, feeding, and mobility
- Monitor and report changes in resident condition to the nursing team
- Accurately document care, vital signs, and intake/output as directed
- Respond promptly to resident needs and call lights
- Support admissions, transfers, and discharges to ensure resident comfort
- Maintain a clean, safe, and calm environment
- Observe and report skin conditions, pressure areas, and safety concerns
- Follow infection control, safety, and confidentiality guidelines
- Work collaboratively with nursing and interdisciplinary teams
- Current Certified Nursing Assistant (CNA) license in good standing in the State of California
- CPR certification preferred
- Compassionate, dependable, and team-oriented
- Strong communication and observational skills
- Commitment to providing respectful, resident-focused care
