** Salary Range:** $45-65/hour (DOE)

** RN Integration Specialist
** to join our rural health clinic team. This key clinical leadership role focuses on advancing care coordination, staff competency, and workflow integration to improve patient outcomes. You will support proactive chronic disease management, standardized clinical practices, and collaborative team-based care across our Davenport, Reardan, and Wilbur clinics.
** Job Responsibilities
*** Collaborating with Providers on care plans for patients with chronic conditions (diabetes, hypertension, COPD, etc.)
* Managing disease registries and coordinating proactive patient outreach
* Providing onboarding, training, and ongoing competency assessments for MAs, MA-Rs, MA-As, and LPNs
* Educating and coaching patients on diagnoses, treatment plans, and self-management goals
* Supporting pre-visit planning, chart preparation, and post-visit follow-ups
* Assisting with Medicare Annual Wellness visits
* Leading and participating in clinical programs, group visits, and quality initiatives
** Qualifications
* ** Graduate of an accredited school of nursing
* Current Washington State RN license
* BLS Certification required
* Minimum 3–5 years of nursing experience in a clinical setting (care coordination, chronic disease management, or case management preferred)
* Strong communication, coaching, and leadership skills
* Proficiency in EMR documentation and clinic-based quality measures
* Certified Diabetes Educator preferred (or willingness to obtain)###
