Job Overview
Full time, 4x10 shifts a week, 7am-5pm (with a 1-hour lunch). No night or weekend rotations. Float primarily to all MO primary care offices with potential KS coverage if needed.
Responsibilities- Cover Advanced Practice Providers (APPs) and Physicians in Primary Care who are absent for extended periods such as FMLA leave.
- Assume responsibility for the absent provider’s patient panel and work within the assigned office, utilizing existing exam rooms and clinical staff to ensure continuity of care.
- Collect and analyze client health data, identify appropriate expected outcomes, develop and implement plans of care, and provide comprehensive clinical coordination and case management.
- Provide consultation to influence plan of care using prescriptive authority procedures and treatments in accordance with state and federal laws and regulations.
- Evaluate client progress in attaining expected outcomes, conduct nursing research, and provide education to patients, families, and nursing staff.
- Deliver comprehensive care to established patients, including history, physical exam, treatment plan, and proper follow-up.
- Adjust schedules and float within the Primary Care division to meet patient needs.
- Participate in continuing education opportunities to maintain proficiency in evidence-based practice.
- Serve as a consultant and leader to clinical staff within the clinic.
- Ensure delivery of clinical services that meet and exceed quality standards, while providing an excellent patient experience.
- Meet or exceed the system’s RVU productivity goals.
- Participate in clinic and system provider meetings to stay informed and encourage communication.
- Complete necessary patient paperwork and respond to in‑basket messages in a timely manner.
- Perform procedures documented on CPA/PASA consistent with education, training, and experience of both the APP and their collaborating physician.
- Develop high‑quality visit notes with efficiency in documentation and utilize critical thinking to identify a need for collaboration with the physician.
- Maintain all requirements for DEA and BNDD if prescriptive authority for controlled medications is necessary.
- Order and interpret diagnostic studies.
- Provide personalized education to patients regarding self‑management, medications, and long‑term complications.
- Leverage the support of population health staff such as social work and pharmacy staff to meet patients’ medical, biopsychosocial, and financial needs.
- Minimum 2 years experience as an Advanced Practice Provider (MO/KS) or equivalent.
- Advanced Practice Provider certification (MO/KS). Various specialties acceptable.
- Basic Life Support certification via Healthstream.
- Master’s Degree in nursing or related field.
- Full Time, Day shift (United States of America).
