Our client is seeking an LPN Team Lead to join their growing team. The LPN Team Lead directs daily operations and program development to promote effective, efficient, responsive, and quality service. This individual participates in multidisciplinary collaboration to perform all services within the LPN’s scope of practice under the direction of the medical practitioners. This individual guides patients by providing support services and encourages engagement in the treatment process (education and integrated support, advocacy, coordination of care, etc.).
This position is currently offering a $2,500 hiring bonus!
- Lead daily operations and guide Medication Management (MM) team members.
- Develop processes to increase departmental efficiencies and aid in implementing solutions.
- Interact and collaborate regularly with practitioners to provide direction and oversight of effective schedules, efficient workflows, and continual improvement.
- Assist patients with enrollment in medication assistance programs and work with Lincoln Regional Center for Legislative Bill 95 (LB 95) patient medications.
- Manage and/or request patient medication refills, medical records, and lab requests.
- Liaise between the patient and practitioner between medication management appointments.
- Assist in patient referral needs as directed by the practitioner.
- Schedule and administer injections as directed by the practitioner.
- Take and document vital signs, lab results, etc.
- Maintain storage of medications, including required monthly counts and documentation.
- Manage process correspondence from pharmaceutical representatives in regard to patient enrollment status/applications and medication sample supply.
- Notify practitioners of patient-reported side effects from medication.
- Document patient correspondence in Electronic Health Record (EHR) system.
- Maintain a positive and respectful demeanor toward all clients and staff in alignment with recovery-focused, trauma-informed care.
- Other job‑related duties as assigned.

- Regular and predictable attendance, and promptness for work.
- Must be a Licensed Practical Nurse in the state of Nebraska.
- At least two years of medical experience are required.
- Leadership experience preferred.
- Knowledge of medications, potential side effects, and co‑occurring medical conditions.
- Demonstrated understanding of and ability to work with people of diverse backgrounds.
- Valid driver’s license, liability auto insurance, and the ability to drive for organizational business.
