GENERAL SUMMARY

Responsible for meeting patient priority needs by assessing, participating in interdisciplinary care planning, providing and/or delegating care, and evaluating patient responses and progress toward clinical outcomes. Contributes to an efficient and effective work environment through professional communication and documentation, precepting, and/or resourcing of others.

Supervises Licensed Practical Nurses and/or other support staff in the delivery of patient care as described in Nebraska Statute 38.

PRINCIPAL JOB FUNCTIONS

  • Initiates assessment and reassessment based on the patient’s presenting problems/needs in accordance with Bryan Medical Center procedures.
  • Synthesizes relevant initial and ongoing patient data to identify nursing priorities and desired outcomes/goals.
  • Integrates appropriate standards of care into the patient’s plan based on diagnosis and patient‑specific needs.
  • Executes therapeutic and/or supportive interventions.
  • Provides education for the patient and family to meet identified needs according to abilities, preferences and readiness to learn.
  • Evaluates the patient’s response to and effectiveness of interventions.
  • Assesses patient progress toward established outcomes.
  • Incorporates cultural considerations into the plan and delivery of care.
  • Includes family, significant other, or identified support person in the care of the patient as appropriate.
  • Documents patient care and related information according to Bryan Medical Center documentation procedures.
  • Provides care in collaboration with other interdisciplinary team members.
  • Facilitates patient transition to the next level of care.
  • Assumes responsibility in a shared or delegated assignment and supports team members’ practices through timely communication.
  • Serves as a role model to colleagues, students, and other care providers.
  • Actively participates in continued learning opportunities and self‑development relevant to the unit and duties.
  • Integrates evidence‑based research findings into practice in accordance with policy and procedure.
  • Engages in departmental and organizational performance improvement initiatives.
  • Appropriately utilizes all safety equipment as defined by organizational procedures.

  • ADDITIONAL JOB FUNCTIONS
    • Provides documentation and care support to a diverse patient population across inpatient units.
    • Evaluates and documents patient responses to nursing interventions, adjusting the plan as needed.
    • Supports the direct patient care team through video monitoring and documentation.
    • Collaborates on patient assessments with bedside nurses and completes documentation.
    • Establishes effective communication plans with bedside nurses and support staff regarding changes in patient condition.
    • Utilizes appropriate nursing diagnoses and assessment data to set realistic outcomes.
    • Documents measurable goals on the plan of care and modifies the plan when indicated.
    • Follows the organization’s chain of command in appropriate circumstances.
    • Collaborates with all healthcare team members, including providers, when planning patient care and documents accordingly.
    • Demonstrates effective interpersonal skills within the interdisciplinary team.
    • Completes the admissions navigation process for scheduled, unscheduled, or direct patient admissions on patient care units and the Emergency Department.
    • Supports the patient discharge process, finalizing orders and medications, scheduling follow‑up appointments, and providing discharge teaching.
    • Assesses patient/family readiness for discharge planning and makes appropriate referrals.
    • Monitors medication administration for safety and performs dual sign‑off when appropriate.
    • Implements and documents nursing interventions consistent with the plan of care.
    • Effectively manages multiple responsibilities, prioritization, urgent responses, and challenging situations.
