Nurse Care Coordinator
Saint Luke's East is seeking a Nurse Care Coordinator to join our team.
Position:
Registered Nurse (RN) – Care Coordinator
Part Time, 24 hours per week. Preferred shifts include Thursday, Friday, Saturday, Sunday, Monday, with a weekend differential applicable for weekend shifts.
Responsibilities- Focus on care transitions through the continuum of care.
- Ensure quality patient outcomes, monitor resource utilization, and mitigate obstacles to care delivery through interdisciplinary collaboration, communication, and coordination.
- Develop and carry out discharge plans and mitigate barriers that impede care progression.
- Assess patient needs and develop individualized discharge plans.
- Identify additional services needed and make appropriate referrals.
- Lead and facilitate multidisciplinary rounds.
- Help ensure clinical readiness for discharge.
- Collaborate with the multidisciplinary team.
- Assist with throughput management and mitigate delays in care.
- In collaboration with the attending physician, identify appropriate length of stay and monitor resource utilization.
- Work with patients, families, and caregivers to ensure patient needs are met and a timely and appropriate discharge plan is implemented.
- Maintain appropriate documentation.
- Valid Missouri RN license.
- Minimum 1 year of Registered Nurse experience.
- Bachelor of Science in Nursing (BSN) required within 3 years of start date.
- Registered Nurse experience validated through the National Council of State Boards of Nursing (NURSYS).
