Job Description
Saint Luke’s East is seeking a Nurse Care Coordinator to join our team. The position is a Registered Nurse (RN) – Care Coordinator, part time, 24 hours per week, day shift.
SchedulePart Time, 24 hours per week, Days
Availability- Thursday, Friday, Saturday
- Friday, Saturday, Sunday
- Saturday, Sunday, Monday
Weekend differential is applicable for weekend shifts.
Job Requirements- A valid Missouri RN license and at least 1 year of RN experience.
- BSN degree required within 3 years of start of position.
- Registered Nurse (MO) – National Council of State Boards of Nursing (NURSYS).
- Assess needs and develop individualized discharge plans.
- Identify additional services needed and make appropriate referrals.
- Lead and facilitate multi-disciplinary rounds. Help ensure clinical readiness for discharge.
- Collaborate with multidisciplinary team. Assist with throughput management and work to mitigate delays in care.
- In collaboration with attending physician, identify appropriate length of stay and monitor resource utilization.
- Work with patients, families and caregivers to ensure patient needs are met, and a timely and appropriate discharge plan is implemented.
- Maintain appropriate documentation.
