Job Description
Saint Luke's East is seeking a Nurse Care Coordinator to join our team.
Position: Registered Nurse (RN) - Care Coordinator
Schedule: Part Time, 24 hours per week, Days
Preferred Day
Shifts:
- Thursday, Friday, Saturday
- Friday, Saturday, Sunday
- Saturday, Sunday, Monday
Weekend differential is applicable for weekend shifts.
Valid MO RN license and 1 year of RN experience required.
The Care Coordinator is responsible for big picture focus of care transitions through the continuum. He/she is part of the multi-disciplinary team ensuring quality patient outcomes, monitoring resource utilization, and mitigating obstacles to care delivery through interdisciplinary collaboration, communication, and coordination. Responsibilities include developing and carrying out discharge plans and mitigating barriers that impede care progression. BSN Degree required within 3 years of start of position.
- Assesses needs and develops individualized discharge plan.
- Identifies additional services needed and makes appropriate referrals.
- Leads and facilitates multi-disciplinary rounds, ensuring clinical readiness for discharge.
- Collaborates with multidisciplinary team; assists with throughput management and works to mitigate delays in care.
- In collaboration with attending physician, identifies appropriate length of stay and monitors resource utilization.
- Works with patients, families, and caregivers to ensure patient needs are met and a timely discharge plan is implemented.
- Maintains appropriate documentation.
Applicable experience: 1 year Registered Nurse (MO) – National Council of State Boards of Nursing (NURSYS). Bachelor’s Degree in Nursing (BSN) required.
