The schedule includes a mix of planned and PRN daytime visits, primarily as part of a weekend rotation, with the opportunity to pick up additional weekday or after-hours on-call work based on interest and availability.

At Hillcrest, our mission is simple:

  • Communicate changes in condition, concerns, and recommendations to providers and interdisciplinary team members.
  • Collaborate with physicians, therapists, social workers, caregivers, and other clinical partners to coordinate care.
  • Complete accurate and timely clinical documentation in accordance with agency standards and regulatory requirements.
  • Support scheduled and PRN daytime visits as part of the weekend rotation.
  • Pick up additional weekday or after-hours on-call visits based on interest and availability.
  • Use sound clinical judgment and problem-solving skills while working independently in the field.
  • Promote Hillcrest Home Health and its mission through professional interactions with patients, families, providers, and community partners.
