SUMMARY GENERAL SUMMARY
Coordinates care for bariatric surgery patients through the continuum pre/post-management of ongoing program accreditation compliance. This includes bariatric program development, patient, and staff education, as well as the oversight of multidisciplinary teams and acting as the liaison between the hospital and surgical practice.
PRINCIPAL JOB FUNCTIONS1.

* Coordinates administrative functions and the development and implementation of patient care clinical pathways; navigates the patient through the process; updates patient policies and procedures for care of bariatric surgery patients.
3.
* Functions as a clinical liaison and resource for the hospital program; responsible for assisting the hospital with achieving and maintaining certification/accreditation through meeting or exceeding standards of care.
4. Assists in the identification and reporting of quality issues, and in the development of a performance improvement plan for the program to assess and improve the quality and efficiency of the program including but not limited to quality assessment, utilization review, risk management, and infection control.
5. Identifies appropriate indicators to monitor and track and in conjunction with marketing, tracks patient data advertising campaigns, and patient referral outcomes.
6. Manages accreditation criteria to ensure clinic/program meets expectations set forth by governing agencies. Maintains and assures compliance of all procedures and policies that guide and support the assigned services.
7. Collaborates with physician leadership, clinic staff and providers, promoting positive communication techniques and the ability to problem solve at every level.
8. Works with others to anticipate and identify the needs of the center and creatively implements solutions to problems. Integrate evidence-based practice into clinic improvements.
9. Provides orientation/education for the center and the hospital staff, students and others as required.
10. Ensures effective staff education, including orientation, focused education, and reinforcement education, to achieve performance outcomes for leading best practice and safe care.
11. Demonstrates effective teaching methods for multi-level individuals and/or groups for delivering effective personalized, patient-centered, family-focused outcomes and professional development of colleagues.
12. Maintains knowledge of documentation requirements needed for patients’ records including knowledge of computer applications.

* Facilitates patient and family education, and the discharge process to promote continuity of care after discharge and optimal patient outcomes; and reviews and revises patient education materials.
14. Develops curriculum and provides skills labs or in-service education for medical staff, employees, or contract staff regarding the special needs of bariatric patients; education may include sensitivity training, lift & transfer techniques, complications of bariatric surgery, post-op care psychosocial needs, etc.
15. Maintains strict confidentiality of Personal Health Information (PHI) following Health Insurance Portability and Accountability Act (HIPAA) policy and government regulation; maintains strict confidentiality of employee information.
16. Utilizes hospital information systems to provide orders/data and retrieve patient information.
17. Maintains knowledge of documentation requirements needed for patients’ records including knowledge of computer applications. 18. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
