POSITION SUMMARY

Provides a variety of support services to homeless or near-homeless Veterans affected by mental illness; assists in coordinating services and communication among specialists and CNCAP; provides financial assistance for eligible persons; identifies and accesses resources; seeks to expand program and resources independently and confidently; promotes and maintains public relations; and completes forms accurately and timely. Provides case management, and assists in recruiting and enrolling Veterans into the H.E.L.P. program.

ROLE AND RESPONSIBILITIES
  • Responsible for completing the agency intake form and inputting it into Clarity
  • Provide ongoing support to veterans and document via home visits, phone calls, and written correspondence with goal attainment of 65% or above
  • Maintain a non-judgmental attitude toward clients and other CNCAP staff
  • Ensure clients are assessed through the grant process to determine possible assistance through resources
  • Create family personal plans to develop short and long-term attainable goals with veterans
  • Ensure files are complete with the necessary paperwork for veteran families participating in the program
  • Adhere to all CNCAP-related policies and procedures
  • Promote the program and agency in a positive and professional manner
  • QUALIFICATIONS
  • High school diploma or GED required (Bachelor's degree in human services or related field preferred)
  • Valid driver's license
  • 1-3 years of experience in human services or related field
  • Must be a Veteran with lived experience
  • SKILLS & ABILITIES

  • Knowledge of Word, Excel, PowerPoint, and databases
  • Excellent time-management and organizational skills
  • Demonstrated ability to work with others
  • SCHEDULE

    Monday-Thursday: 7 am - 5 pm or Flexible

    JOB STATUS

    Full-Time, 40 hours per week

    PAY

    $19.50-$20.50 (depending on qualifications)
