Principal Job Functions
  • Client Services
  • Engages in and develops trusting relationships with clients
  • Respect the range of diverse cultures and the broad array of needs experienced by individuals and families.
  • Assists the family in developing an individualized support network.
  • Provides services to clients in their home or community setting.
  • Maintains active and regular contact with clients, including at least one face-to-face contact per month.
  • Engages clients in developing their goals.
  • Provides parent educational/skill building and/or emotional support to families.
  • Supports clients with information, tools, and resources they need to develop new competencies and become their own advocates.
  • Works cooperatively with program partners to secure housing and related support services for clients.
  • Connects clients to needed mainstream and community resources.
  • Provides client transportation, using personal vehicle.
  • Provides regular updates to referral sources or other involved parties.
  • Participates actively in supervision to better meet the needs of clients.
Requirements REQUIRED

KNOWLEDGE, SKILLS AND ABILITIES:
  • Knowledge of community resources and case management best practices, including Motivational Interviewing and evidenced-based housing models and related support services.
  • Ability to work with a variety of individuals from diverse backgrounds.
  • Ability to work cooperatively and professionally with community partners and representatives from other agencies.
  • Ability to work independently yet utilize supervision as needed.
  • Ability to drive and provide proof of driver's license and auto insurance.
EDUCATION AND EXPERIENCE:
  • Bachelor's Degree in a related field.
  • Minimum of twelve months of previous experience working with individuals and families from diverse backgrounds.
