Delivery of patient care

• Personal care (ADL) including bathing (assist, bed bath, tub bath, shower, sitz), body mechanics (turn, position, transfer, use lifts, ambulation, ROM exercises), nutrition (prep meals, feed, measure intake/output, prepare snacks), elimination (bedpan/urinal, bowel/bladder retraining, specimens, perineal/catheter care, etc.), safety (side rails, restraints, CPR/Heimlich, infection control), special procedures (vital signs, temperature, pulse, respirations, BP, height/weight, heat/cold application, decubitus ulcers care, skin prep, dressing, ace bandages, TEDs, binders, postmortem care), housekeeping, participation in interdisciplinary team conferences.

NA 1+ 4 responsibilities

• Sterile dressing for wounds >48 hrs, wound irrigation
• Ostomy care or irrigation
• Insertion of urinary catheter limited to replacement of existing catheter after nursing assessment
• Oxygen therapy including setup and monitoring

NAII responsibilities

• Breaking‑up and removal of fecal impaction
• I.V. Fluid; assisted activities
• Nutrition activities (oral/nasogastric infusions, gastrostomy feedings, tube clamping/removal)
• Suctioning (oropharyngeal, nasopharyngeal)
• Tracheostomy care
• Insertion of urinary catheter limited to replacement of existing catheter after nursing assessment
• Sterile dressing and wound irrigation
• Ostomy care or irrigation
• Oxygen therapy with setup and monitoring

• Attends and participates in interdisciplinary team conferences (IDT) and special care conferences
• Assumes responsibility for effective working relationships with team members
• Identifies patient/caregiver needs and refers to appropriate team members

• Ensures accurate, complete, succinct, and timely clinical documentation per HPCCR guidelines and licensure requirements

• Demonstrates competency in knowledge and skills
• Maintains HPCCR professional reputation
• Engages in professional development; stays current on industry trends
• Maintains licensure, CPR certification, CEU requirements
• Maintains professional relationships with external agencies and providers
• Exhibits time‑management; plans workday and responds to crises appropriately

• Patient room cleaning per procedure; ensures all areas are clean and stocked
• Maintains family room cleanliness throughout day
• Ensures proper chemicals and protective equipment usage in cleaning

• High School diploma preferred; training/certification in related field required
• Certified Nursing Assistant; current listing status required
• Must be in good standing with NC Department of Health and Human Services (for NA)

• Minimum 1 year experience required
• Experience in home care, working with terminally ill and/or geriatrics preferred
