Overview
Our client is seeking experienced Nurse Practitioners to perform in-home health assessments for Medicare and Medicaid members. This is an independent, field-based role requiring strong clinical judgment, excellent communication skills, and the ability to work in a wide variety of home environments. The role offers scheduling flexibility and requires availability of at least 10 hours per week.
Key Responsibilities- Perform in-home health assessments for Medicare Advantage members
- Work independently in a field-based setting
- Communicate effectively with members in a variety of home environments
- Apply strong clinical judgment during assessments
- Active, unencumbered Nurse Practitioner license
- Minimum 6 months of Nurse Practitioner experience
- No new graduates
- Ability to work independently in the field
- Strong clinical judgment
- Excellent communication skills
- Availability for 10+ hours per week
- Adult Gerontology Primary Care NP
- Adult NP
- Family NP
- Gerontology NP
- Acute Care NP, accepted only in specific states
- Active NP license, unencumbered
- National Board Certification required: AANP or ANCC
- BLS/CPR required
- ACLS not accepted
- Mental Health board certification not accepted
- Pediatrics board certification not accepted
