Job SummaryFamilyway Home Care is seeking a compassionate and experienced Perdiem Case Manager / Registered Nurse (RN) to join our team. This role is responsible for coordinating patient care, ensuring compliance with state and federal regulations, and providing oversight to home health aides. The Case Manager/RN will serve as the primary point of contact for clients and families, developing and implementing care plans to meet each client's unique needs.
Key Responsibilities Patient Care Coordination- Assess, develop, and oversee individualized care plans for clients receiving home care services.
- Conduct initial and ongoing assessments of patients to evaluate their health status and home care needs.
- Monitor and ensure the quality of care provided by home health aides.
- Provide education to patients and families on health conditions, medications, and self-care techniques.
- Maintain accurate and timely clinical documentation in accordance with state, federal, and agency guidelines.
- Ensure that home health services comply with Medicaid, Medicare, and private insurance and Joint Commission regulations
. - Conduct regular home visits to assess patient progress and ensure adherence to care plans.
- Train, supervise, and evaluate home health aides to ensure they follow care plans and provide high-quality care.
- Work closely with physicians, therapists, and other healthcare professionals to coordinate and adjust care as needed.
- Serve as a liaison between the agency, clients, families, and healthcare providers.
- Identify and address patient safety concerns
, ensuring a safe home environment for clients. - Participate in case conferences and quality assurance initiatives to improve patient outcomes.
- Assist with audits and regulatory inspections as required.
- Active RN license in the state of New Jersey.
- Minimum of 2 years of nursing experience
, preferably in home health, case management, or a related field. - Strong knowledge of Medicaid, Medicare, Joint Commission and private insurance guidelines for home healthcare services.
- Experience in care planning, patient assessment, and interdisciplinary coordination
. - Excellent communication and organizational skills
. - Ability to work independently while managing multiple patients and priorities.
- Proficiency in electronic medical records (EMR) systems
.
- Experience with geriatric, post-surgical, or chronic disease management
. - Familiarity with state and federal home health regulations
. - BLS/CPR certification
.
