What You'll Do
- Provide skilled nursing care including medication administration, symptom management, wound care, and ongoing patient monitoring.
- Observe changes in patient condition and communicate with the RN Case Manager and interdisciplinary team.
- Educate patients, caregivers, and families about medications, comfort measures, and the hospice plan of care.
- Build lasting relationships through regular home visits in private homes, assisted living communities, memory care, and skilled nursing facilities.
- Complete timely and accurate EMR documentation.
- Participate in weekend, holiday, and on-call rotations as scheduled.
What You'll Bring
- Active Minnesota Licensed Practical Nurse (LPN) license.
- Previous experience in hospice, home health, long-term care, skilled nursing, oncology, or palliative care required.
- Strong communication and relationship-building skills.
- Ability to work independently while collaborating closely with the interdisciplinary hospice team.
- Compassion, reliability, and a commitment to exceptional patient-centered care.
