Role ResponsibilitiesSupport patients in Downers Grove and surrounding areas; collaborate with the care team to manage and deliver patient care plans.
Compensation & BenefitsEligible for $20,000 total bonus potential, including a $10,000 first‑year success bonus and an additional $10,000 sign‑on bonus paid within the first 18 months of employment. Bonuses earned upon meeting performance expectations and remaining in the role and status for which hired.

- Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college
- Licensed Physical Therapist issued by the state in which you will be working
- Minimum of one year healthcare experience
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
- A commitment to consistently meet critical deadlines for charting
- The skills needed to self‑manage your time and schedule
- Demonstrated experience with tablets, mobile phones and EMR software
