Hospice Registered Nurse, RN

Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice for a nursing job in Lisle, Illinois.

This ongoing position supports patients in Lisle, IL and surrounding areas and requires collaboration to provide management and delivery of patient care plans.

• Collaborate with interdisciplinary team to manage and deliver patient care plans.
• Support patients and families throughout the hospice care continuum.
• Maintain accurate charting and documentation to meet critical deadlines.
• Utilize tablets, mobile phones, and EMR software in daily practice.
• Provide after‑hours support as part of the 24/7/365 care team.

• RN license in the state of employment.
• Associate degree or higher from an accredited School of Nursing.
• Minimum two years of RN experience; hospice experience preferred.
• Valid driver’s license and ability to travel ~20% of the day to patient locations.
• Proficiency with EMR software and mobile devices.
• Commitment to meeting charting and documentation deadlines.
• Proficiency in Russian or Spanish is a plus.
