Residential Home Health and Hospice seeks dedicated Hospice Registered Nurses to collaborate in the management and delivery of patient care plans for residents in Lisle, IL. The role supports patients and their families through comprehensive, team‑based care.

• Collaborate with interdisciplinary team to develop and implement patient care plans.
• Provide direct bedside and transportation care for hospice patients.
• Ensure accurate and timely charting to meet critical deadlines.
• Assist with patient and family education regarding hospice services and care plans.

• Active RN license in the state of employment.
• Associate degree or higher from an accredited School of Nursing.
• Minimum two years of RN experience, with hospice experience required.
• Current driver’s license and ability to drive approximately 20% of the day to patient locations.
• Commitment to consistently meet critical deadlines for charting.
• Successful completion of a pre‑employment drug screen (including evaluation of medical and recreational marijuana).
