Position Overview

We are seeking a compassionate Hospice Registered Nurse to support patients in the home setting. As part of our team‑based care model, you will collaborate to manage and deliver patient care plans.

Key Responsibilities
  • Provide hospice care and support to patients in Lisle, IL and surrounding areas.
  • Collaborate with the care team to develop and implement individualized care plans.
  • Document patient status and care actions in the electronic medical record (EMR) in a timely and accurate manner.
  • Coordinate with physicians, therapists and other professionals to ensure comprehensive care.
  • Educate patients and families about care plans and hospice services.
  • Travel to patient locations, spending approximately 20% of your day driving.
  • Self‑manage scheduling and workload to meet critical charting deadlines.
Qualifications
  • RN license in the state of employment.
  • Associate degree or higher from an accredited nursing school.
  • Minimum of two years of RN experience, with hospice experience required.
  • Valid driver’s license and reliable transportation.
  • Strong written and verbal communication skills.
  • Proficiency with tablets, mobile phones and EMR software.
  • Ability to independently manage time and schedule.
