Position Summary

As a Wound Care focused Nurse Practitioner
, you will provide hands‑on care inside senior communities, managing complex wounds while building ongoing relationships with your patients. This role is ideal for someone who values independence, variety, and the ability to directly influence patient outcomes
. You will serve as a key clinical partner to staff, families, and physicians within your assigned communities.

Responsibilities
  • Provide on‑site rounding and advanced wound care services
  • Develop and manage individualized treatment plans
  • Collaborate with physicians and interdisciplinary teams
  • Communicate with families and community staff
  • Manage medications, labs, and documentation
Qualifications
  • Active Certified Nurse Practitioner (CNP) license in Minnesota
  • Experience in geriatrics, primary care, or wound care preferred
  • Valid driver’s license and reliable transportation
  • Passion for serving the senior population
