Job Overview

Registered Nurse – Home Health position at Elara Caring provides high‑quality patient care in the patients’ homes.

• Ensure all activities align with the vision of Elara Caring’s leadership.
• Maintain patient and staff privacy and confidentiality pursuant to HIPAA.
• Assess assigned case load, identify patients’ physical, psychosocial, and environmental needs.
• Assume primary responsibility for caseload: assess, plan, coordinate, implement, and evaluate the plan.
• Integrate data on physical, psychological, social, and spiritual factors into the nursing care plan.
• Conduct initial and follow‑up evaluations, adjusting visits per patient, family, and facility needs.
• Communicate significant findings or changes to the Clinical Supervisor, physicians, and other team members.
• Revise the plan of care in response to identified issues and notify the team.

• Current, unrestricted state RN license.
• 1+ year experience in a clinical care setting.
• Willing and able to travel within the coverage area; dependable vehicle, valid driver’s license, and auto insurance.
• Associate’s or Bachelor’s degree in Nursing preferred.
• Previous Home Health or Hospice experience preferred.
• Experience with Home Care Home Base preferred.

• Base salary $75,000–$87,000 annually, determined by experience, education, and equity
