License Practical Nurse LPN Home Health PRN

Job Description

At Elara Caring, we provide high‑quality home‑care services to more than 60,000 patients daily. We are looking for a Licensed Vocational/Practical Nurse to help deliver care in patients’ homes and support them on their health journey.

• Verify the Plan of Care prior to each visit and provide care according to delegation by the RN, physician’s orders, assessment data, and established standards and guidelines.
• Incorporate patient care goals established in the Plan of Care into nursing visits, as documented in skilled nursing notes.
• Communicate significant findings, problems, and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care.
• Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician as required.
• Implement the plan for patient safety using the patient, family, and community resources.
• Assist patients in learning appropriate self‑care techniques while delivering quality patient care.
• Suggest revisions to the plan of care in response to identified patient care issues and notify the RN Case Manager.
• Provide services that require substantial and specialized nursing skills under the direction of a Registered Nurse or physician.
• Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions.
• Document medication regimen and update medication profile according to established procedures.
• Treat patients and caregivers with the highest and most effective level of care.
• Implement all available actions to prevent avoidable hospitalizations and ER visits.
• Provide positive, supportive communication to physicians, patients, families, visitors, and agency personnel.

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• Current State License as a Licensed Vocational/Practical Nurse.
• 1 year of experience in a clinical care setting.
• Experience in a hospice or home health environment is preferred.
• 50% travel required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver’s license and insurance and reliable transportation to perform job tasks.
